Surgery of eyelids and adnexa

BLEPHAROPLASTY

Blepharoplasty, some times intended as a face MINI-LIFTING, has developed into one of the most widely spread cosmetic proceedings for both men and women, thanks to the latest laser kinds and to the patient’s high satisfaction.
Skin resurfacing with CO2-ERBIUM laser has proved to be an highly efficient method since it allows a better control of the produced wound depth. It has widely replaced both deep chemical peeling and skin scraping off methods.
Blepharoplasty can be performed on both upper and lower eyelids.

BEFORE OPERATION
Before the treatment both internal and external eyes health conditions shall be carefully evaluated. In particular the following aspects shall be analyzed: visual quality, tears release, use of contact lenses, use of particular drugs; these aspects shall be discussed together with individual needs and expectations.
These information together with other elements such as the patient’s age, kind of skin and "background" will allow a safer and more satisfactory surgical choice.

THE OPERATION
Blepharoplasty is usually performed through local anhestesia. A slight opiate can be administered for allowing the patient to relax. The operation times will be different according to the amount of tissue to be removed.
There is a difference between upper and lower blepharoplasty. Basically, for the upper one, the cut shall be practiced above the skin and therefore externally, while for lower blepharoplasty the cut is directly made on the inside conjunctiva. This difference is substantial concerning the wound healing. In fact, the upper operation shall require the use of stitches which are not needed for the lower one. However the cuts are practiced along the natural folds so that once healed it is difficult, when not impossible, to see them after some time.

AFTER OPERATION
Edema and swelling are normal reactions and generally their lasting can range from one week to one month after operation.
Cold pads and adequate masks will help to reduce edema and swelling. Ocular ointments and collyria can be administered for keeping eyes wet and clean. Generally, the post-operation discomfort is slight even if it can be different from a patient to another. After the operation, the patient is supplied with a list, where all the necessary useful aspects he has to learn concerning his post-operation time are indicated, such as hygienic recommendations and therapy. In case of use of stitches, they are usually removed on the fifth day, while the "reabsorbable" ones will disappear by themselves.
It is very important to meet all the post-operation requirements in order to accelerate healing and reduce the risks of complications.

RESULTS
The experience of most of patients subjected to this process by ERBIUM or CO2 laser ray is a real improvement of their eyes aspect. Besides that, most of them says to feel more industrious and energetic. But despite the positive effects of blepharoplasty are immediately evident, the result is considered decisive only 1 year after, up to the end of the collagen neoformation process.

SIDE EFFECTS
Normally after a face complete resurfacing about 10 days are needed without working and under adequate therapy. In fact, generally the treatment produces INFLAMMATION, REDNESS and more or less enduring EDEMA. A collateral effect can be an abnormal pigmentary answer of the treated tissue with HYPERPIGMENTATION or HYPOPIGMENTATION. But nowadays there is a really small amount of these troubles thanks to the adoption of the CO2-ERBIUM combined laser.
